In-Home Care

CCAP In-Home Care has been providing services for over 25 years. We currently serve approximately 300 clients in Grays Harbor and Pacific Counties. We provide these services by employing over 117 caregivers.

Our Services enable the elderly and the disabled to remain at home for as long as possible, in many cases stemming off the need for expensive nursing home stays. We also provide services to children and people with developmental needs. Services vary according to the needs of the client, including:

Housework, Laundry, Personal Care, Meal Preparation, Medication Reminders

Transportation to physician appointments, 24-hour Care.

This is a fee-for-service program. The majority of our clients qualify for state funded programs. Less than 20% of our clients pay with private funds.

Medical Transportation

Transportation is provided to residents of both Grays Harbor County and Pacific County. There are three different transportation programs that serve Seniors, low-income residents, the disabled, and elderly who do not drive.

Medicaid Medical Transportation for residents who use DSHS Coupons. Call Paratransit Services at 1-800-846-5438 for information. for the hearing impaired, call TTY 1-800-934-5438

Cancer Treatment Transportation for anyone unable to reach radiation treatment in Olympia. Call CCAP at 360-533-5100 or 1-800-828-4883 for information on this program.

Private Medical/Basic Needs Transportation for Seniors over age 60 who no longer drive due to health or aging issues. All Private Medical/Basic Needs Transportation is provided by volunteers who use their own vehicles or use a program van. All drivers are trained and qualified before accepting rides. HIV/AIDs case management and services

Services are provided via the Ryan White Program. This program has a support group that gets together at least four times a year.Case Management services may include supporting access to: HIV medical care, Social Support Groups, Educational Programs, transportation for medical appoitments, Client Advocacy, one-on-one case management and participation in Grays Harbor Annual AIDs Walk.

Housing opportunities for people with HIV/Aids:

This program provides rental, mortgage and utility assistance to individuals with HIV/AIDS. Case management services and supportive services are available through the HIV/AIDS Care Consortium

Personal Hygiene Packs

CCAP provides individuals and families with personal hygiene items. A household is eligible every 6 months to receive these items. Hygiene Packs include a toothbrush, toothpaste, shampoo, soap, laundry soap and other items.

Long-Term Care Ombudsman Program

This program recruits, trains and retains volunteers to advocate for residents of long-term care facilities. The volunteers are assigned to facilities and visit their residents weekly.

Social Security Representative Payment Program

This program serves individuals that receive Social Security and/or SSI payments that cannot manage their funds. Our main responsibility as a representative payee is to use the funds to pay for the current and foreseeable future needs of the beneficiary. We currently serve about 105 beneficiaries through this program.

Senior and Caregiver Peer Counselors

Respond to senior community members in crisis who do not fit a mental health diagnosis. Peer Counselors are listening specialists trained by local professionals who are also on call to assist and advise the Peer Counselors if necessary. Peer Counselors assist 25 to 30 residents annually.
