Health Services


In-Home Care

CCAP In-Home Care has been providing services for over 25 years. We currently serve approximately 300 clients in Grays Harbor and Pacific Counties. We provide these services by employing over 117 caregivers.

Our Services enable the elderly and the disabled to remain at home for as long as possible, in many cases stemming off the need for expensive nursing home stays. We also provide services to children and people with developmental needs. Services vary according to the needs of the client, including:

Housework, Laundry, Personal Care, Meal Preparation, Medication Reminders, Transportation to physician appointments, 24-hour Care. We are also a distributor for Philips LIFELINE.

This is a fee-for-service program. The majority of our clients qualify for state funded programs. Less than 20% of our clients pay with private funds.

                             Caregiver Training

CCAP In-Home Care provides training opportunities for both Revised Fundamentals of Care Giving(Basic Training) and Continuing Education. In addition to partnering with the Training Partnership, which administers all union-sponsored caregiver training, CCAP is also the only state registered "Community Trainer" in this region.

As a Community Trainer CCAP is working to support the training needs of local adult family homes, boarding homes, DDD homes and home care aides (IPs). We provide training via custom or specialized classes for non-union caregivers on an as-needed basis. Arrangements can be made for training sessions to be provided either at CCAP or on-site at a group facility.

S.A.I.L

The Aberdeen Senior Center offers exercise classes "Stay Active In Life" on Wednesdays and Fridays, 10-11 am in the Aberdeen Museum of History next to the Senior Center. The group focuses on fall prevention and the cost is $2. For more information call 360-533-5100 x141

 

Medical Transportation

Transportation is provided to residents of both Grays Harbor County and Pacific County. There are three different transportation programs that serve Seniors, low-income residents, the disabled, and elderly who do not drive.

Medicaid Medical Transportation for residents who use DSHS Coupons. Call Paratransit Services at 1-800-846-5438 for information. for the hearing impaired, call TTY 1-800-934-5438.

Cancer Treatment Transportation for anyone unable to reach radiation treatment in Olympia. Call CCAP at 360-533-5100 or 1-800-828-4883 for information on this program.

Private Medical/Basic Needs Transportation for Seniors over age 60 who no longer drive due to health or aging issues. All Private Medical/Basic Needs Transportation is provided by volunteers who use their own vehicles or use a program van. All drivers are trained and qualified before accepting rides. HIV/AIDs case management and services

Services are provided via the Ryan White Program. Case Management services may include supporting access to: HIV medical care, Social Support Groups, Educational Programs, transportation for medical appointments, Client Advocacy, one-on-one case management and participation in Grays Harbor Annual AIDs Walk.

 

Personal Hygiene Packs

CCAP provides individuals and families with personal hygiene items. A household is eligible every 6 months to receive these items. Hygiene Packs include a toothbrush, toothpaste, shampoo, soap, laundry soap and other items.

Long-Term Care Ombudsman Program

This program recruits, trains and retains volunteers to advocate for residents of long-term care facilities. The volunteers are assigned to facilities and visit their residents regularly. For volunteer opportunities or for questions Please call 360-533-5100 x120

Social Security Representative Payment Program

This program serves individuals that receive Social Security and/or SSI payments that cannot manage their finances. Our main responsibility as a representative payee is to use the funds to pay for the current and foreseeable future needs of the beneficiary. We currently serve about 180 beneficiaries through this program.

         Senior Peer Counselors

The Senior Peer Counseling Program is designed to assist seniors through a natural life crisis. A life crisis could be anything from having trouble moving through the grieving process after the loss of a loved one or pet, dealing with illness, change of residence, or other major life changes.

Senior Peer Counselors can help clients find new support systems, solve their own problems and assist participants in finding new places to live.

Peer Counselors don't do In-Home Care services or housework. They are not a Senior Companion, a moving company, or a transportation service.
