Case Management services are available to people living with HIV/AIDS

Services may include supporting access to:

• HIV medical care
• Social Support Groups
• Educational Programs
• Transportation for medical appointments
• Client Advocacy
• One-on-one case management
• Participation in Grays Harbor 's Annual AIDS Walk

Eligibility Requirements

To become eligible for HIV/AIDS case management services, you must:

• Have documented proof of HIV/AIDS status
• Have a need for case management services

Housing Opportunities for People with Aids

This program provides rental, mortgage and utility assistance to individuals with HIV/AIDS. Case management services and supportive services are available through the HIV/AIDS Care Consortium.
