SUMMARY DESCRIPTION: This position is expected to greet the public and handle multiple duties at one time and complete them accurately and timely.  This position is expected to have working knowledge of agency programs and local resources to provide accurate information to the public.

HOUSING AND COMMUNITY SERVICES

Care Coordinator, Aberdeen Office

SUMMARY DESCRIPTION: This position provides comprehensive care coordination for individuals experiencing homelessness or housing instability. The Care Coordinator works with participants to develop individualized support plans to achieve housing stability and move toward self-sufficiency. The Care Coordinator interacts with diverse community supports, including behavioral health, substance abuse treatment, physical health, criminal justice, benefits management and employment.

This is a full-time, hourly position. The position is based in Long Beach WA, but may occasionally require travel to and work in our South Bend and Aberdeen offices.

Youth Shelter Advocate, On Call

SUMMARY DESCRIPTION: The Youth Advocate directly supports youth voluntarily staying at a HOPE youth shelter in Aberdeen, WA. The shelter provides temporary residence, assessment, referrals, and permanency planning services for unsheltered youth under the age of 18.
This position will play a critical role in assuring the shelter’s efficient operation, working to sustain a safe environment and healthy culture needed to support successful stays and exits for youth utilizing the shelter.

Shelter Advocate

SUMMARY DESCRIPTION: This position supports persons staying overnight at a seasonal cold weather shelter located in Aberdeen, WA. The shelter supports adults aged 18+, daily between 9 pm and 7 am. The shelter operates for five months from the beginning of November through the end of March. This position plays a critical role in assuring the shelter’s efficient operation, working to create the safe environment and healthy culture needed to support successful stays and exits for persons utilizing the shelter.

Bilingual Care Coordinator, Long Beach Office

SUMMARY DESCRIPTION: This position provides comprehensive care coordination for individuals experiencing homelessness or housing instability. The Care Coordinator works with participants to develop individualized support plans to achieve housing stability and move toward self-sufficiency. The Care Coordinator interacts with diverse community supports, including behavioral health, substance abuse treatment, physical health, criminal justice, benefits management and employment.

In addition to these duties, the Bilingual Care Coordinator specializes in outreach and engagement with Spanish-speaking individuals and communities. The position provides interpretive services and consultation to ensure cultural competency for all programs at CCAP.

This is a full-time, hourly position. The position is based in Pacific County WA, working from our Long Beach and South Bend offices. The position requires community outreach in Pacific County, and may occasionally require travel to and work in our Aberdeen office.

SUMMARY DESCRIPTION:  This position carries out community-based case management and “support services” for individuals experiencing employment barriers due to factors including; substance abuse, mental illness and/or chronic physical illness. This is a full-time position located in Aberdeen, Washington.
