Floor Staff – HOPE youth shelter

SUMMARY DESCRIPTION:  The Shelter floor staff directly supports youth voluntarily staying at a HOPE youth shelter in Aberdeen, WA.  This position will play a critical role in assuring the shelter’s efficient operation, working to sustain a safe environment and healthy culture needed to support successful stays and exits for youth utilizing the shelter.  Must be 21 years of age or older to be employed.  The on-call position requires flexibility and the ability to respond to various schedule changes,  filling in for employee vacations as well as call-outs due to sickness. The shelter provides temporary residence, assessment, referrals, and permanency planning services for unsheltered youth under the age of 18. Open until filled.

LOCATION/WORKING HOURS:   The HOPE Youth Shelter is located in Aberdeen, WA.  The shelter operates continuously, 7 days per week, 24 hrs per day. This position is subject to pre-employment drug testing and requires an FBI background check.

care coordinator Runner – Grays Harbor

Summary Description: The Care Coordinator Runner is a pilot position within the Housing & Community Services (HCS) department, based in Grays Harbor County. This is a utility role designed to provide flexible, cross-program support by serving as a peer transport and engagement resource for participants enrolled in HCS programs. The Care Coordinator Runner is available to participants whose services are approved under Foundational Community Supports (FCS) Housing and works across all HCS programs to provide reliable, person-centered transportation and light engagement support.

This position is funded through FCS Housing billing and is designed to be self-sustaining through consistent billable activity. The Care Coordinator Runner plays a critical role in connecting participants to appointments, services, and supports that promote housing stability and overall well-being. HCS Administrative support ii – Grays Harbor county

SUMMARY DESCRIPTION: The HCS Administrative Support II provides full-time administrative support to Care Coordinators, Program Specialists, and leadership across CCAP’s Housing & Community Services programs in Grays Harbor, Pacific, and Lewis Counties. This position is based at the Aberdeen office and manages a broad range of participant-focused administrative tasks that reduce documentation burden on direct service staff, ensure program compliance, and support continuity of participant services.

Core functions include recertifications, reauthorizations, benefits administration, FCS application support, assignment tracking, HEN subsidy coordination, and logistical participant support. This position does not carry a direct client caseload but works in close coordination with Care Coordinators and program staff to support participant outcomes. Weatherization and energy assistance program coordinator – grays harbor County

SUMMARY DESCRIPTION: This position supervises CCAP’s Weatherization and Energy Assistance Team.  The Coordinator is responsible for managing workflow and production; negotiating and managing sub-contracts and supervising the work product of other team members.  Working with the Department Director and the agency’s Fiscal team, the Coordinator develops and manages an overall program budget, ensuring that all costs are coded to the appropriate funding source and category, and are within the limits of available funding.  The Coordinator works closely with funders, and prepares reports to funders.

energy auditor/ crew electrician – grays harbor County

Ryan White case manager – grays harbor county

SUMMARY DESCRIPTION: This position provides advocacy, assessment, case management, information, referral and outreach for eligible program participants. Works closely with the primary health care providers, case managers, state, and local agencies who provide supportive services for HIV positive individuals. Re-entry care coordinator II – Lewis County

SUMMARY DESCRIPTION: This position provides comprehensive care coordination and Coordinated Entry (CE) services for individuals experiencing substance use disorder, frequent and lengthy terms of incarceration, homelessness, or housing instability. The Care Coordinator works with participants to develop individualized support plans to achieve housing stability and move toward self-sufficiency.

This position is part of the Lewis County Coordinated Entry system, and conducts CE eligibility screenings, diversion conversations, and full CE assessments with individuals experiencing homelessness or housing instability, including individuals currently incarcerated in the Lewis County Jail who are preparing to reenter the community.

The Care Coordinator interacts with diverse community supports including: behavioral health, substance use treatment, physical health providers, criminal justice partners, benefits systems, and employment services. The position operates under a braided service model combining Coordinated Entry activities with care coordination funded through Foundational Community Supports (FCS) and the Pathways Community HUB.

This is a full-time position based in Lewis County, WA. The position includes regular work within the Lewis County Jail and frequent travel throughout the county to conduct field visits, home visits, and community coordination. Open until filled.

Employment Lead worker- Pacific County –

SUMMARY DESCRIPTION: This position will be primarily stationed in Pacific County and serve as a lead worker primarily for Pacific County with support to Grays Harbor County. This position will be providing assistance to the Employment Coordinator for all employment programs by coaching and providing onsite assistance and training for all employment staff. You also will regularly conduct audits of services provided to ensure contract compliance and ensure that information and training are communicated with employment staff.  You will assist the Employment Coordinator in preparing for onsite contract compliance visits and fill in to provide employment services at times.Open until filled.

Landing Pad Care Coordinator II

SUMMARY DESCRIPTION:   The Landing Pad Care Coordinator II is a direct-service position within the Landing Pad program, a voluntary short-term transitional housing program operated by Coastal Community Action Program (CCAP) in partnership with the Clark County Jail Programs Unit. The Landing Pad provides immediate safe housing, peer support, intensive case coordination, and community connection for individuals being released from the Clark County Jail who are navigating behavioral health challenges, housing instability, and re-entry barriers. Services are available 24 hours a day, 7 days a week, 365 days a year.

The Care Coordinator II works directly with participants from the moment of intake through program exit, supporting individualized re-entry planning, connection to behavioral health and community services, and successful transition to longer-term housing and support within 14 to 30 days. This position uses lived experience in the justice system as a core practice tool for building trust, supporting recovery, and helping participants connect to the resources they need.

This is a full-time, hourly position. Staff are assigned to one of three shifts: Day (6a–3p), Swing (2p–11p), or Overnight (10p–7a), working five days per week with two consecutive days off. All shifts include a one-hour unpaid meal break. The Landing Pad is located at the Center for Community Health in Clark County, WA. Open until filled.

 Landing Pad Care Coordinator II – Float

SUMMARY DESCRIPTION:  The Landing Pad Care Coordinator II (Float) performs all of the same direct-service duties as the Landing Pad Care Coordinator II and is additionally scheduled to provide first-call coverage for shift callouts. The Float Care Coordinator is assigned a primary shift and works four scheduled days per week. On their three off-schedule days, the Float Care Coordinator is not on-call and is not required to be available. They serve as the first contact when the supervisor is seeking coverage for an unplanned absence.

Shifts accepted on available days are paid at the standard hourly rate and will not exceed 40 paid hours per week without supervisor authorization.
This role is well suited for someone who values schedule flexibility, is comfortable working across multiple shift contexts, and thrives in a role that requires adaptability and broad program knowledge.

The Float Care Coordinator plays a critical role in ensuring the Landing Pad meets its contractual two-person minimum staffing requirement at all times.
